Why we collect your personal information

We collect your personal information (including health information) to provide you with our new patient forms service to facilitate your appointment with a health practitioner, and for other purposes which are authorised or required by law.

We collect your name, date of birth, contact details (address, email address and contact number), gender / birth sex, Medicare details, Pension, Health Care Card, or Veterans Affairs number (if applicable), next of kin and emergency contact details, cultural background, allergies and medications, and smoking and alcohol history at the time that you use the new patient forms service, usually via our website or mobile applications.

Who do we disclose your personal information to

We will disclose your personal information to the health practitioner that you have selected and their practice for the purpose of facilitating your health appointment and / or the health practitioner’s provision of a health-related service to you.

We may also disclose your personal information to our third party service providers, but only for the purpose of providing goods or services to us.  We require our third party service providers to agree to appropriate privacy restrictions, and only permit them to access personal information to the extent needed to provide goods or services to us.  Some third party service providers may store your personal information on servers located overseas.  We may also disclose your personal information where authorised or required by law.  We may also disclose de-identified information of our users to third parties for analysis, research and quality assurance purposes.

If you do not want us to collect your personal information

You are under no obligation to provide us with your personal information, however if you do not we will not be able to provide our new patient forms service to you.
